National Application Form for Ethical Review of a Research Project


Information about the national application form for researchers applying for ethical review.

Unless a study qualifies for expedited review or does not require ethics committee review under the Ethical Guidelines for Observational Studies: Observational Research, Audits and Related Activities, the National Application Form should be used to apply for ethical review and a locality assessment provided.

Important: It is highly recommended that researchers refer to the Guidelines for Completion of the National Application Form (particularly the guidelines on writing information sheets and consent forms on page 15) before completing it.

Guidelines for Completion of the National Application Form


These Guidelines provide researchers with practical advice for filling out applications for ethical review.

In order to ensure that your application will be considered, you are strongly encouraged to read and become familiar with them.

See page 1 for the number of copies required and page 21 for the information sheet and consent form guidelines.

Note: A Part 4 signature page is required for each site.

All applications must be typed. Hand written applications will be not be accepted.

There is also a locality assessment form that must be filled out for each of the proposed study localities. Refer to the Guidelines for Completion of the National Application Form pages 19 - 20 for information about when to complete a locality assessment form by ethics committee. In addition, if your research involves:
Use of human tissue, you will also need to submit NAF Part 5.
Genetic research, you will also need to submit NAF Part 6.
Xenotranplantation, you will also need to submit NAF Part 7.
Participants who are unable to give informed consent to participate, such as children or unconscious people, you will also need to submit NAF Part 8.

Examples of Studies to be Submitted on National Application Form


Examples of studies to be submitted on the national application form for researchers applying for ethical review include studies involving:
  1. clinical intervention including extra blood samples or biopsies for the purpose of the study. Extra questioning or use of written questionnaires is not considered clinical intervention. (see the Ethical Guidelines for Observational Studies: Observational Research, Audits and Related Activities, NEAC, Sept 2006, paragraph 11.7)
  2. specimen collection, storage and use except for the exceptions outlined in the Ethical Guidelines for Observational Studies: Observational Research, Audits and Related Activities, NEAC, Sept 2006, paragraph 11.8 (also complete Part 5 of the application form)
  3. genetic testing (also complete Part 6 of the application form)
  4. in-depth interviews
  5. use of Established Human Embryonic Stem Cell Lines (refer to the National Co-ordinator for additional information on submitting an application).
