1. Applications to the Multi-region Ethics Committee

Please refer to the guidelines for completing the application form and ensure the following: Please note that incomplete or incorrectly collated applications may be returned to the sender at their expense.

2. Delivering Applications to the Upper South Committees
There is no longer a receptionist at the Christchurch office, level 4, 250 Oxford Terrace, to receive applications. A 'drop box' system is in operation. The dimensions of the drop box are 46cm by 20cm. If you do not wish to leave a larger box/package in the public foyer area, please ensure that it is in packages that will fit into the drop box.


The 2009 version of the National Application Form for Ethical Approval of a Research Project is now available. The 2005 version will be accepted until 30 September 2009. Researchers may be asked, however, to resubmit the compensation declaration form B if it is not the 2009 version and provide other documentation requested in the application form 2009-v1. The committees would like to thank researchers and organisations for their input into the process.

Note: a checklist is to be attached to the front of each application. Incomplete applications will not be considered. Pending is an option only for written confirmation of Maori consultation, SCOTT approval, and Locality assessment by organisation. For multi-region studies, the documentation for one site must be complete.
