The 2009 version of the National Application Form for Ethical Approval of a Research Project is now available. The 2005 version will be accepted until 30 September 2009. Researchers may be asked, however, to resubmit the compensation declaration form B if it is not the 2009 version and provide other documentation requested in the application form 2009-v1. The committees would like to thank researchers and organisations for their input into the process.

Note: a checklist is to be attached to the front of each application. Incomplete applications will not be considered. For multi-region studies, the documentation for one site must be complete.
