Change of Name of Upper South B Administrator from Diana Passe to Diana Whipp
Our congratulations to Diana Whipp (previously Passe) on her recent marriage. Please note her change of surname.

National Application Form for Ethical Approval of a Research Project-2009-v1
The 2009 version of the National Application Form for Ethical Approval of a Research Project is now available. The 2005 version will be accepted until 30 September 2009. Researchers may be asked, however, to resubmit the compensation declaration form B if it is not the 2009 version and provide other documentation requested in the application form 2009-v1. The committees would like to thank researchers and organisations for their input into the process.
Note:
a checklist is to be attached to the front of each application. Incomplete applications will not be considered. Pending is an option only for written confirmation of Maori consultation, SCOTT approval, and Locality assessment by organisation. For multi-region studies, the documentation for one site must be complete.
